Quality Improvement Experience
iMPROve Health has been driving quality improvement projects since 1984. These projects are not just limited to the state of Michigan and have a national scope. We are always looking for new partnerships to help improve the health of Michiganders and beyond. Together, we can make the greatest impact. American Heart Association
Centered on year-long interventions at federally qualified health centers (FQHCs), the EmPowered Clinicians program assisted providers in better identifying high blood pressure in patients using the Measure Accurately, Act Rapidly, and Partner with Patients (M.A.P.) framework. The program also gave participating FQHCs the tools to better support their patients in self-monitoring their blood pressure. iMPROve Health co-led this program with the American Heart Association (AHA).
We supported five FQHCs by offering blood pressure training to staff, self-measured measured blood pressure (SMBP) training, and providing pre-, mid-, and post-assessments of their implementation of the M.A.P. framework.
Outcomes:
- All FQHCs saw an improvement in accurately performing blood pressure measurements.
- Baseline assessment score: 58%
- Final assessment score: 79%
- All FQHCs saw an improvement in patient responses to SMBP after receiving training.
- Baseline assessment score: 23%
- Final assessment score: 88%
Southeast Michigan Beacon Community
iMPROve Health was chosen to partner in the Southeast Michigan Beacon Community (SEMBC). SEMBC was one of 17 Beacon Communities throughout the country. The coalition helped to strengthen the local health information technology infrastructure. The initiative was funded by the Office of the National Coordinator for Health Information Technology. SEMBC used technology to reduce the devastating effects of diabetes in its region by making measurable improvements in health, care, costs, and innovative testing approaches.
SEMBC’s major partners included but were not limited to:
Blue Cross Blue Shield of Michigan, Beaumont Health System, Centers for Medicare & Medicaid Services Region V Office, Community Health and Social Services Center, Detroit Medical Center, Ernst & Young, The Ford Motor Company, General Motors, Greater Detroit Area Health Council, Michigan Department of Community Health, Michigan State Medical Society, and Michigan State University.
Breast, Cervical, and Colorectal Cancer Prevention
Under the direction of the Michigan Department of Health and Human Services, iMPROve Health promoted and enhanced the well-being of Michiganders by improving the rates for cancer prevention screenings and early detection.
Meaningful Use
Michigan Center for Effective Information Technology Adoption (MCEITA) and Small Underserved and Rural (SURs) Project
iMPROve Health assisted healthcare providers with achieving the Centers for Medicare & Medicaid Services Electronic Health Record Incentive Program, also known as Meaningful Use. We helped providers optimize the quality, efficiency, and security of their health information technology. Altarum was the main fiduciary for both projects.
Physician Quality Reporting Initiative (PQRI) and Physician Quality Reporting System (PQRS)
We were selected to promote, educate, and assist individual and eligible professional groups to meet and report to this Centers for Medicare & Medicaid Services incentive program. Eligible professionals were given the opportunity to assess the quality of care they delivered to be sure each patient received the right care at the right time.
