Another Successful ECHO Project

Project Echo logoThe Social Determinants of Health ECHO for Care Teams ended  with participants reporting they felt very knowledgeable and 81% of participants would ‘probably’ or ‘definitely’ use what they learned. iMPROve Health hosted the virtual ECHO series to help primary care teams adopt screening for social determinants of health  (SDOH) and address specific needs.

Didactic topics presented by iMPROve Health and external subject matter experts (SMEs) included: SDOH in Michigan, SDOH impact on blood pressure and cholesterol, self-measured  blood pressure, screening for SDOH, SDOH Z codes, following up on SDOH, statewide  resources, and SDOH and telehealth.

“A valuable piece of the ECHO model is the chance for peer-to-peer learning,” said Julee Campbell, MPH, CPHQ, senior quality consultant with iMPROve Health. “Participants were able  to discuss questions and barriers with their peers and could draw on the group’s variety of  perspectives for brainstorming solutions.”

Participants had opportunities to learn more about SDOH, health-related social needs and other  relevant topics. They gained knowledge, resources, and new perspectives to bring back to their  teams and use when screening patients for SDOH and connecting them with appropriate  resources.
